Law Financial Aid Forms

We are unable to accept any forms that include an electronic signature, all forms must have an actual signature. Additionally, pictures of documents will not be accepted.

my.UChicago Secure Upload

If you are a current student or an admitted student with a claimed CNetID, we encourage you to upload any documents to the Financial Aid Office using your my.UChicago portal. This is a secure method of sharing sensitive documents with us and is the preferred method for sending us information as an admitted or current student.

  • Once logged into your my.UChicago portal, navigate to "My Financial Aid", click "Upload Law Aid Materials", choose the applicable academic year from the dropdown menu, and follow the instructions on the "Upload Law Aid Materials" interface.
  • If you have supporting documentation for an appeal, combine the appeal form and all supporting documentation into the same file for upload. Uploading multiple files for the same appeal or submission will delay the awarding process.
  • Your documents will be uploaded to your account and we will review and link them to your financial aid file, usually within 2-3 business days. 

If you cannot use the upload process or are a prospective student or alumni who needs to submit documentation, contact our office for additional guidance.

General Forms

Use this optional form if you want to authorize the Financial Aid Office to discuss your financial aid information with a third party individual, such as your spouse, parent, etc. We will not release information specific to your financial assistance without your written permission.

Completion of this form does not allow the Financial Aid Office to release federal tax information (FTI) from your FAFSA. FTI is any federal tax information that is transferred directly from the IRS into your FAFSA as a result of your consent and disclosure to transfer this data into your FAFSA through direct data exchange.

Use this optional form if you need the Financial Aid Office to share federal tax information (FTI) from your FAFSA with an outside, third-party organization for the purpose of awarding financial aid, including outside scholarships. FTI is any federal tax information that is transferred directly from the IRS into your FAFSA as a result of your consent and disclosure to transfer this data into your FAFSA through direct data exchange.

University Payroll Forms
From time to time, the University will require certain tax forms (W-9, UPP192, W-8BEN, etc) to process payment. If Financial Aid or University Payroll requests these forms, you must upload them to your Workday profile. More information about uploading documents to Workday is on the UChicago GRADHelp website.
